Bring Us Your Benefit Card…We’ll Take Care of the Rest!

Insurance Direct Billing Service 

Custom Bodies Fitness and Massage offers a Direct Billing service. Insurance policies are all different in what they cover for Massage, so we require that you contact your Insurance Company to get a good understanding of the type of coverage you have.

​These are some questions you should ask your insurance provider:

  • Does my benefit plan allow for direct billing by my massage therapist?
  • Do I require a prescription, from my attending physician, for massage therapy?
  • When does my year start for coverage?
  • How much coverage do I have for Massage Therapy alone?
  • Is there a maximum amount that I get per massage and if so, how much is it?
  • We only bill to the primary insurance company, so any co-ordination of benefit plans will be your responsibility.

*If, for whatever reason, you insurance provider rejects the direct billed claim you will be responsible for the outstanding amount.

As of June 30, 2018 we will be completely set up to direct bill to all of the following and more:

  • Medavie Blue Cross (Federally Administered programs for the Canadian Armed Forces (CAF), Royal Canadian Mounted Police (RCMP) and for Veterans Affairs Canada (VAC))

No Show & Cancellation Policy – We require 24 hours notice to change, cancel and/or reschedule Massage Therapy appointments. If you need to cancel or change your scheduled appointment, please notify us 24 hours prior to your scheduled time by phone or email. If 24 hours is not given, the client will be responsible to pay for the full price of the scheduled appointment. 

MVA (motor vehicle accident) claim: Following a motor vehicle accident, you might be eligible to claim your massage therapy treatment under your bodily injury policy. Contact your motor vehicle insurance adjustor and your primary healthcare provider (ie: physician, chiropractor or physiotherapist)  before book your appointment.
